Your name is simply for identification. Current email is where your wizwar.com email will be forwarded to. Requested email is what you would like your new email address to begin with. Password is needed incase you ever wish to change your email forwarding. For example, I would enter 'Dave' as my name, 'dave@dugancom.com' as my current email, and 'dave' as my requested email. Currently you have two choices for an extension, wizwar and wiz-war. Please pick only one. Also note that there will never be a need for you to 'check' this email as it will be automatically forwarded to your current email address and show up in your regular email software.
